This Healthcare Applicant Agreement (the “Agreement”) is entered into by and between the applicant/client (“Applicant”) and Continent Post Inc., including its subsidiaries such as Continent Post Healthcare (collectively referred to as “Continent Post”), for the purpose of authorizing the submission and renewal of insurance applications with the New York State Marketplace.
1. Authorization to Submit Insurance Application
The Applicant authorizes Continent Post Inc. and its subsidiaries, including Continent Post Healthcare, to act on their behalf in submitting an insurance application to the New York State Marketplace. This authorization includes providing all necessary personal information required to complete the application process.
2. Authorization for Annual Insurance Renewal
The Applicant authorizes Continent Post Inc. and its subsidiaries, including Continent Post Healthcare, to annually renew their insurance policy with the New York State Marketplace. This includes the use of the Applicant’s personal information to facilitate the renewal process.
3. Use of Personal Information
The Applicant acknowledges and consents to Continent Post Inc. and its subsidiaries, including Continent Post Healthcare, using their personal information solely for the purpose of submitting insurance applications and processing annual renewals with the New York State Marketplace.
4. Duration and Revocation
This Agreement shall remain in effect until terminated by the Applicant. The Applicant may revoke this authorization at any time by providing written notice to Continent Post via email at support@continentpost.com. Revocation will not affect actions already taken under this Agreement prior to receipt of the notice.
5. Privacy and Security
Continent Post will take all reasonable measures to protect the Applicant’s personal information in accordance with applicable laws and regulations. The Applicant understands that their personal information will be shared only as necessary for the purposes outlined in this Agreement.
6. Acknowledgment and Acceptance
By clicking “I Agree” on the application, the Applicant acknowledges that they have read and understood the terms of this Agreement and voluntarily agree to its provisions. Clicking “I Agree” serves as an electronic signature, binding the Applicant to the terms outlined herein.
